What the codes mean
The small print at the bottom of the Commander 2013 card (#254).
254
- 254Collector number: this card's place in the set list. Main-set cards print it over the set's card count.
Cards from 1998 to mid-2014 print the collector number over the set's card count (like 90/383) next to the copyright line. They have no rarity letter, set code or language code: the set symbol and its colour show the set and rarity.

You choose whether to add or remove a counter when the second ability resolves. You can’t choose to remove a counter if there isn’t one there.
2005-10-01
If the third ability triggers, removing a counter in response won’t stop the effect. However, somehow removing Plague Boiler from the battlefield in response would stop the effect because then you wouldn’t be able to sacrifice Plague Boiler.
2005-10-01
The sacrifice is done on resolution of the triggered ability.
